OHVIRA (Obstructed Hemivagina and Ipsilateral Renal Anomaly) syndrome is a rare congenital disorder that affects the female reproductive and urinary systems. It is characterized by the presence of a septum that divides the vagina into two parts, with one side being obstructed, and the other being normal. The condition is often associated with renal anomalies on the same side, such as renal agenesis or duplication.OHVIRA can lead to various symptoms, such as pelvic pain, dysmenorrhea, recurrent infections, and infertility. Treatment typically involves surgical correction of the vaginal anomaly and management of any associated renal anomalies.Irrespective of imaging modality, the typical findings are:duplication of the uterus, cervix and vaginaunilateral hematocolpos or hematometrocolposcongenital renal anomalies on the same side as the uterovaginal obstruction (most likely renal agenesis, and less likely multicystic dysplastic kidney and renal duplication) Gartner duct cystspelvic endometriosisHerlyn-Werner-Wunderlich (HWW) syndrome, is a rare anomaly characterized by Müllerian duct anomalies (MDA) associated with mesonephric duct anomalies . This entity is also known as obstructed hemivagina and ipsilateral renal anomaly (OHVIRA). It is not to be confused with the Wunderlich syndrome (a spontaneous non-traumatic subcapsular and/or perirenal space hematoma).
